DAY TREATMENT PROGRAM FOR NEUROLOGICAL REHABILITATION

The NeuroMedical Center’s Outpatient Day Neuro Program exists to provide an intense, interdisciplinary, collaborative, functional approach of therapy to individuals with neurological impairments so as to maximize their independence and participation in their home, community, work, educational, and social roles.

WHY DAY NEURO?

After hospitalization following a neurologic episode, one of the most valuable steps a patient can take is to participate in our Day Neuro Program.  The program exists to provide treatment beyond inpatient rehabilitation and to help ease the transition from the inpatient or acute care setting to life back at home. Our innovative outpatient services are aimed at improving:

  • Safety & Independence in Daily Activities

GOAL-ORIENTED PROGRAM

The treatment team works individually with each patient to achieve the following goals:

  • Provide an individualized assessment of patient’s prior roles, goals, and current functional performance
  • Create an integrated interdisciplinary plan for achievement of individual’s and/or family’s goals
  • Increase an individual’s performance and functioning through the use of neuromuscular reeducation techniques which promote motor learning and recovery
  • Incorporate family and/or caregivers through education, training, guidance, and support so as to maximize an individual’s progress toward goals and maximize their performance of daily activities
  • Provide emotional adjustment and coping skills to create an environment of success for each patient
  • Utilize therapeutic activities in real life environments through structured tasks and skills to assist with transitioning to community centered goals
  • Provide appropriate discharge planning and implementation for each individual

DAY NEURO CANDIDATES

Individuals who have experienced a life changing neurological incident resulting in a change in function are appropriate to attend the Outpatient Day Neuro Program at The NeuroMedical Center.
