The NeuroMedical Center partners with Craniometrix to connect eligible patients and their caregivers with GUIDE, a free Medicare program that covers the costs of dementia care navigation services.
GUIDE stands for Guiding an Improved Dementia Experience. The program provides personalized guidance, practical resources and ongoing support to help families feel more informed and confident throughout the care journey.

Who Is Eligible for the GUIDE Model?

Who the GUIDE Program Supports

GUIDE is designed for dementia patients and their family members or caregivers supporting them.

The program may be helpful for:

Patients with dementia

Patients living with dementia, Alzheimer’s disease or another dementia

Family Members

Family members helping coordinate a loved one’s care

Caregivers

Caregivers who need guidance, resources or additional support

Support Built Around Your Needs

Each patient and family is paired with a dedicated Care Navigator trained by Johns Hopkins. Your Care Navigator will take the time to understand your priorities and create a plan tailored to your situation.

  • A Personalized Care Plan

    Your Care Navigator will help develop a plan centered on your health, daily needs, personal goals and caregiving priorities.

  • Expert Guidance

    Receive practical support for navigating daily life, future planning and other important considerations that may arise as memory changes progress.

  • Help Whenever You Need It

    A 24/7 helpline provides patients and caregivers with access to support at any time.

  • Caregiver Resources

    Eligible participants may receive up to $2,500 per year for professional caregiving services, including in-home support.
